What to Bring

To ensure a smooth examination, please bring the following items:

Valid driver's license or government-issued photo ID
Current list of all medications with dosages
Glasses or contact lenses if used for driving
Hearing aids if applicable
Medical records for any chronic conditions
CPAP machine settings if using for sleep apnea
Doctor's clearance letters if applicable
